Ethambutol + Isoniazid + Rifampicin Adverse Reactions / Ethambutol + Isoniazid + Rifampicin Side Effects

GI disturbances, psuedomembranous colitis (rare), abnormalities of liver function, fatalities in those with liver disorders, influenza -like symptoms, skin reactions, eosinophilia, transient leucopaenia, thrombocytopaenia, shock, drowsiness, headache, ataxia, visual disturbances,menstrual irregularities. Reddish coloured urine & tears. Peripheral neuritis, optic neuritis; psychotic reactions, convulsions, nausea, vomting fatigue, epigastric distress, visual disturbances, fever, rash, pyridoxine deficiency. Retrobulbar neuritis with a reduction in visual acuity, constriction of visual field, central or peripheral scotoma & green- red coloured blindness. Retinal haemorrage (rare); reduced renal clearence of urates (acute gout); rash, headache, dizziness, confusion, hallucinations, malaise, jaundice; thrombocytopenia; pulmonary infiltrates. Hepatitis (rare); fatalities following liver necrosis, hepatotoxicity.

Special Precautions

Hepatic & renal imapirment. Check liver function before treatment. Body fluids may colour orange- red. Discontinue use if thrombocytopaenia or purpura occurs. Monitor blood counts. Convulsive disorders; history of psychosis; patients at risk of neuropathy or pyridoxine deficiency e.g diabetic, alcoholic, malnourished, ureamic, infected with HIV, Impaired pre- treatment visual acuity. Elderly, malnourished or very young patients, alcoholic patients. Pregnancy, & lactation.

Other Drug Interactions

Liver enzyme induction affects metabolism of chloramphenicol, corticosteriods, clofibrate, ciclosporin, dapsone, digoxin, hexobarbitone, ketoconazole, quinidine & verapamil. Antagonises hypoglycaemic action of insulin. Pyridoxine antagonises INH effects, absorption delayed or reduced by Al hydroxide, Synergistic effect with other antitubercular agents. Blood levels lowered by phenobarb & phenytoin. Increase in blood levels with probenecid. Decreases effects of tolbutamide, clofibrate, coumarin anticoagulants, diazepam & oral contraceptives. Reduced levels of metoprolol, norethisterone, quinidine. Reduced vit D blood levels. Severe hepatitis when combined  with isoniazid. Reduced efficacy of corticosteroids in Addison's disease & induces an addisonian crisis. Increases effect of toxicity with alcohol.

Hypersensitivity; jaundice, biliary obstruction, severe hepatic disease, previous history of hepatic damage during INH therapy; IM/ SC route of administration; porphyria; optic neuritis, severe renal failure; history of epilepsy, children < 6 years.
