P - Caution when used during pregnancy

As a surgical aid, Sodium hyaluronate is indicated in IOL implantation procedures, retinal attachment surgery, corneal transplantation, intra and extracapsular extraction and glaucoma filtration. During anterior segmental surgical procedures, Sodium hyaluronate can reduce corneal endothelium trauma.
Sodium hyaluronate can prevent post-operative flat chamber formation by pushing back the vitreous face. Sodium hyaluronate can create a clear field for intra and post-operative inspection of photocoagulation and retina.

Sodium hyaluronate is a polysaccharide which functions as a tissue lubricant. It is widely used in ophthalmic surgery because it forms a viscoelastic solution in water which makes it a suitable substitute for aqueous and vitreous humour.

Widely distributed in body tissues and intracellular fluids.

Excretion
Ophthalmic, via Canal of Schlemm.

Sodium hyaluronate can cause transient increase in IOP, liritis, hypopyon, corneal edema and corneal decompensation.

Precautions

Sodium hyaluronate is contraindicated in aphakic diabetic individuals and ocular infection patients. Do not overfill Sodium hyaluronate in anterior and posterior segment of the eye.

Monitor IOP continuously while instilling Sodium hyaluronate. Avoid trapping of air bubbles.
Do not use Sodium hyaluronate solution if cloudy particles are present.

Dosage

For IOL implantation and cataract surgery (Adults):
Administer Sodium hyaluronate injection using a cannula before or after lens implantation.
 
For glaucoma filtration surgery
After trabeculectomy, inject Sodium hyaluronate carefully into the corneal paracentesis to fill the anterior chamber. Continue the injection until extrusion into the sub-conjuctival filtration site and suture the outer scleral flap.

For retinal attachment surgery
Sodium hyaluronate should be administered slowly into the vitreous cavity. The drug can be employed for safe excision and to separate epiretinal membranes from the retina. The drug can be used to maneuver the tissues into the desired position.
 
Corneal transplantation
The anterior chamber should be filled with Sodium hyaluronate after removing the corneal button and continue the procedure.
