Sevelamer is a copolymer of 2-(chloromethyl)oxirane (epichlorohydrin) and prop-2-en-1-amine. The marketed form sevelamer hydrochloride is a partial hydrochloride salt being present as approximately 40% amine hydrochloride and 60% sevelamer base. The amine groups of sevelamer become partially protonated in the intestine and interact with phosphorus molecules through ionic and hydrogen bonding.



Sevelamer hydrochloride is a polymeric compound. It acts by binding to phosphate molecules in the gut, limiting its absorption and thus lowering serum phosphate levels without altering calcium, aluminum, or bicarbonate concentrations.

Sevelamer Hydrochloride Adverse Reactions / Sevelamer Hydrochloride Side Effects

Headache, infection, pain, hypertension, hypotension, thrombosis, diarrhoea, flatulence, dyspepsia, nausea, vomiting, constipation and cough. Pruritus, rash and abdominal pain.


Sevelamer does not contain calcium or alkali supplementation. Monitor serum calcium, bicarbonate, and chloride levels. Monitor for reduced vitamins D, E, and K (clotting factors), and folic acid levels.

Risk of systemic toxicity is low as it is not absorbed from the gut.

Special Precautions

Dysphagia, swallowing disorders, severe GI motility disorders, or major GI tract surgery. Pregnancy and lactation. Monitor serum levels of calcium, bicarbonate and chloride. To be taken with meals and tablets should be swallowed intact.

Other Drug Interactions

Concurrent admin decreases the absorption of ciprofloxacin. Not to be taken concurrently with oral medications in which the safety and efficacy would be significantly affected by its reduced bioavailability.

Adult: Initially, 800-1600 mg tid, taken with each meal and based on serum phosphorus level: 5.5 to <7.5 mg/dL: 800 mg tid; 7.5 to <9 mg/dL: 1.2-1.6 g tid; ≥9 mg/dL: 1.6 g tid. Usual maintenance: 0.8-4 g with each meal. Patients switching from calcium acetate based on current dosage: 667 mg calcium acetate is equivalent to 800 mg sevelamer (carbonate/HCI).
