Beclometasone dipropionate or beclomethasone dipropionate, also referred to as beclometasone, is a potent glucocorticoid steroid. In the form of an inhaler (e.g. Becotide, Qvar), a wide number of brands of which are available, it is used for the prophylaxis of asthma. As a nasal spray (e.g. Beconase, Vancenase), it is used for the treatment of rhinitis (e.g. hayfever) and sinusitis. In some instances it is used by oral pathologists in the treatment of unusually severe canker sores. As a cream or ointment (trade name Propaderm) it is used to treat severe inflammatory skin disorders (e.g. eczema) unresponsive to less potent steroids, but is generally avoided in the treatment of psoriasis due to the risk of rebound on withdrawal.

Beclometasone controls the rate of protein synthesis, depresses the migration of polymorphonuclear leukocytes, fibroblasts, reverses capillary permeability and lysosomal stabilisation at the cellular level to prevent or control inflammation.

Readily absorbed after local application.

Rapidly distributed to all body tissues.
Metabolism
Mainly hepatic but also in other tissues including GI tract and lungs.
Excretion
Chiefly in the urine and as metabolites in the faeces.

Loss of skin collagen and SC atrophy; local hypopigmentation of deeply pigmented skin; dryness, irritation, epistaxis, rarely ulceration or perforation of the nasal septum; smell and taste disturbances; hoarseness and candidiasis of the mouth or throat.

Dosage
Topical/Cutaneous
Corticosteroid-responsive dermatoses Adult: Apply a 0.025% cream/ointment onto affected area.
Nasal
Treatment and prevention of allergic and non allergic rhinitis Adult: 100 mcg bid or 50 mcg 3-4 times daily in each nostril. Max: 400 mcg daily.
Inhalation Prophylaxis of asthma Adult: Initially, 600-800 mcg daily. Maintenance: 400 mcg daily in 2-4 divided doses. Child: As dipropionate: 50 or 100 mcg bid-qid or 100 or 200 mcg bid.
Inhalation
Severe asthma Adult: 250 mcg four times daily or 500 mcg bid. May be increased to 500 mcg 3-4 times daily if necessary. Max: 2 mg daily.
Special Populations
When switching from oral to inhalation steroid for stable asthmatics. Initially, the inhalation is added to existing oral steroid dosage. After 1 wk, oral dose is reduced by 2.5 mg prednisone or its equivalent. Similar reduction is made at wkly intervals depending on patient response. Reinstatement of oral steroid may be needed in times of stress or exacerbation of asthma. Inhalation of β-agonists should be done 5 min before the steroids.

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available.
