Penicillin G sodium is indicated for the treatment of penicillin-susceptible organisms causing wound infections, septicaemia, pneumonia, diphtheria, peritonitis, meningitis, arthritis, osteomyelitis, pericarditis, bacterial endocarditis, genital infections and empyema.
Penicillin G sodium is effective for fusospirochaetosis, uncomplicated gonorrhoea, syphilis, tetanus, listeriosis, rat-bite fever, anthrax, clostridial infections, Lyme’s disease and gonorrhoeal endocarditis.

Pharmacodynamics

Penicillin G sodium exert anti-bacterial action by inhibition of bacterial cell wall polysaccharide biosynthesis.

Pharmacokinetics

After parenteral administration, the drug is completely absorbed, and Cmax can be achieved within 15 – 30 minutes. After absorption, the drug is poorly absorbed in cardiac valves and bones and moderately distributed in body fluids. Penicillin G sodium can cross the placental barrier. The Vss of Penicillin G sodium is about 0.3 to 0.41/kg in adults and approximately 0.751/kg in pediatric patients. About 55% of the administered dose is bound to plasma proteins. The plasma half-life is about 30 minutes. About 50-80% is eliminated via the urine as unchanged drug.

Some of the commonly reported adverse events of Penicillin G sodium are blood dyscrasias, allergic reactions, electrolytic imbalance, stomatitis, diarrhoea, antibiotic-related pseudomembranous colitis and diarrhoea, hepatitis, cholestasis, nephropathy, localized reactions and diuresis. However, the side effects of Penicillin G sodium may vary from one person to another.

Precautions

Penicillin G sodium is contraindicated in individuals with an allergy to penicillin and cross-reactivity to cephalosporins.

A hypersensitivity test should be performed, and the test results should be observed for thirty minutes after administration of the test dose. If skin reactions are observed, do not administer Penicillin G sodium. Discontinue Penicillin G sodium therapy in case of suspected or confirmed antibiotic-associated pseudo-membranous colitis.
Caution should be exercised while treating patients with bronchial asthma, diabetes, epilepsy, hepatic damage, heart disease, acute lymphocytic leukemia and heart disorders.

Dosage

Penicillin G sodium can be administered either as IM or IV injections. The recommended dosage is 30, 000 IU/kg body weight in 2-3 divided doses.
High doses (Adults):
Consider administration of 10-40 million IU/day as IV injection

Normal doses (Adults):
Consider administration of 1-5 million IU/day as either IM or IV injection

For infants up to 10 kg and children up to 12 years:

High doses:
Consider administration of 0.1 to 0.5 (up to 1.0) million IU/day as IV injection

Normal doses:
Consider administration of 0.03 to 0.1 million IU/day as IV or IV injection

For Neonates and premature infants with body weight up to 3.5 kg

High doses:
Consider administration of 0.2 to 0.5 million IU/day as IV injection

Normal doses:
Consider administration of 0.03 to 0.1 million IU/day as IV or IM injection
