Bromelain, a naturally occurring phytoenzyme is present in pineapple and pineapple juice. Bromelain is generally prescribed for the treatment of osteoarthritis, inflammation, hay fever, ulcerative colitis, debridement and shortening of labor. Bromelain can help muscle relaxation, improve antibiotic absorption and prevent cancer.
Bromelain is generally available as Bromelain with trypsin and rutin formulations.

Pharmacodynamics

Bromelain reduces inflammation by interfering with the enzymatic synthesis involved in Arachidonic acid metabolic pathway. Inhibition of such enzymes results in increased bleeding time and prevention or reduced inflammation. Bromelain can interfere with growth of malignant cells by unknown mechanisms.

Bromelain Adverse Reactions / Bromelain Side Effects

Bromelain can cause bleeding tendency, easy bruising and increased clotting time. In certain cases, Bromelain can cause diarrhoea and other GI discomfort.

Precautions

Bromelain is contraindicated in patients who are allergic to pineapple, carrot, fennel, grass pollen, cypress pollen, celery and synthetic Bromelain products/supplements. Bromelain should not be given to patients who need to undergo surgery within two weeks. Stop taking Bromelain at least two weeks prior to a scheduled surgery to avoid bleeding risks.

Bromelain may interact with amoxicillin, tetracycline group of antibiotics, anticoagulants, anti-platelet drugs including NSAIDs.

As a phytoproduct, Bromelain may interact with angelica, aniseed, ginger, asafoetida, celery, chamomile, alfalfa, fenugreek, willow, red clover, licorice, chestnut, poplar, quassia, zinc, potato and soybean.
