Ertapenam sodium, an anti-bacterial drug, is indicated for the treatment of drug-resistant, susceptible bacterial infections including of complicated intra-abdominal infections due to Bacteroides thetaiotamicron, Clostridium clostridioforme, Bacteroides fragilis, Bacteroides ovatus, Escherichia coli, Eubacterium lentum, Bacteroides distasonis and Peptostreptococcus spps. Ertapenem sodium is prescribed for the treatment of complicated skin and skin structure infections including non-osteomyelitis diabetic foot infections due Escherichia coli, Prevotella bivia, Methicillin-susceptible Staphylococcus aureus, Klebsiella pneumonia, Bacteroides fragilis, Peptostreptococcus spps and Porphyromonas asaccharolytica.
Ertapenem sodium is also prescribed for the treatment of Streptococcus pneumoniae induced community-acquired pneumonia with co-infections including Hemophilus influenza and/or Moraxella catarrhalis. Ertapenem can be considered for the treatment of complicated UTI including pyelonephritis due to Klebsiella pneumoniae and/or E. coli. Acute pelvic infections due to post-operative gynaecological infections, postpartum endomyometritis and septic abortions due to S. agalacitae, Prevotella bivia, E. coli, Bacteroides fragilis and Porphyromonas asaccharolytica can be treated by Ertapenem sodium.
Ertapenem sodium is prescribed for prophylaxis of elective, colorectal surgical site sepsis.

Pharmacokinetics

The mean bioavailability of Ertapenem intramuscular injections is approximately 90% with a Tmax of 2.3 hours. Ertapenem is highly bound to plasma albumins. The Vss of Ertapenem is 0.12 L/kg in adults with varying levels in pediatric patients. The predominant drug metabolite is inactive ring-opened derivative. Ertapenem is mostly metabolized via kidneys with mean plasma half-life and clearance of 4 hours and 1.8 L/hour, respectively. Ertapenem is mostly eliminated via the urine as unmetabolized drug.

Some of the adverse events of Ertapenem sodium are watery diarrhoea, loss of coordination, fever, tremors, seizures, muscular stiffness, skin rashes, vaginal itching, headache, diarrhoea, nausea and injection-site reactions.

Precautions

Ertapenem administration is contraindicated in patients with renal diseases, history of traumatic head injury or brain tumor, seizures, allergy to lidocaine or novocaine.

Dosage

Intra-abdominal Infection (Adults):
For complicated infections, 1 gm Ertapenem sodium should be given either as IV or IM, once daily for 5 - 14 days

Pelvic Infections (Adults):
Administer 1 gm of Ertapenem sodium, IV or IM, once daily for 3 - 10 days

Pneumonia (Adults):
For the treatment of community-acquired pneumonia, consider 1 gm of Ertapenem sodium, IV or IM, once daily, for 10-14 days. If positive outcomes are achieved, parenteral therapy can be switched over to oral therapy after three days.

Pyelonephritis (Adults):
Consider 1 gm of Ertapenem sodium, IV or IM, once daily, for 10-14 days. If positive outcomes are achieved, parenteral therapy can be switched over to oral therapy after three days.

Skin or Soft Tissue Infection (Adults):
Consider 1 gm of Ertapenem sodium, IV or IM, once daily, for 7-14 days that can be extended up to 4 weeks. If positive outcomes are achieved, parenteral therapy can be switched over to oral therapy after three days.

Urinary Tract Infection (Adults):
For complicated cases, consider 1 gm of Ertapenem sodium, IV or IM, once daily, for 10-14 days. If positive outcomes are achieved, parenteral therapy can be switched over to oral therapy after three days.

Infection Prophylaxis (Adults):
For the prevention of surgical site sepsis following elective colorectal surgery, consider administration of 1 gm, IV one time, 1 hour prior to surgery.
 
Intra-abdominal Infection (Complicated):
3 months to 12 years: Administer 15 mg/kg, IV/IM, twice daily. The maximum daily dose should not exceed 1 g/day.
 
13 years or older: Administer Ertapenem sodium, 1 gm, IV/IM, once daily. The duration of treatment should be 5-14 days.

Pelvic Infections (Adults):
Consider administration of 1 gm, IV/IM, once daily. The duration of treatment should not exceed 3-10 days.

Pneumonia (Community-acquired):
3 months to 12 years: Consider administration of 15 mg/kg, IV/IM, every 12 hours. The maximum daily dose should not exceed 1 gm. The duration of Ertapenem sodium should be 10-14 days.

USFDA pregnancy category B. Ertapenem sodium may not cause harm to an unborn foetus. Before Ertapenem sodium treatment, the patient should discuss with the physician, if they are planning for a pregnancy.

Ertapenam sodium and Lactation

It is unclear whether the drug could pass through the breast milk to a breast feeding baby. Consult your physician before taking Ertapenem sodium if you are nursing mother.
