Domperidone (trade names Motilium, Motillium, Motinorm and Costi) is an antidopaminergic drug, developed by Janssen Pharmaceutica, and used orally, rectally or intravenously, generally to suppress nausea and vomiting. It has also been used to stimulate lactation.

Domperidone Adverse Reactions / Domperidone Side Effects

Drowsiness, extrapyramidal reactions, galactorrhoea, gyneacomastia; constipation or diarrhoea, lassitude, decreased libido, skin rash, itch. Convulsions, arrhythmias & cardiac arrest, dysrrhythmia in patinents with CV disease or hypocalcemia, patients on cancer chemotherapy, Seizures; hypertensive crisis in patients with phaeochromocytoma.

Phaeochromocytoma; children <2yrs, elderly; renal or hepatic impairment. Risk of cardiac arrhythmias & hypokalemia if administered IV. Pregnancy & Lactation.

Other Drug Interactions

Reduces abosrption of digoxin. Increases abosrption of aspirin, paracetamol & oral diazepam. Enhances CNS depresssion by phenothiazine. Antimuscarinic agents & opioids antagonise GI effects.
May antagonise hypoprolactinaemic effect of drugs such as bromocriptine. Increased effects when used    with CYP3A4 inhibitors such as erthromycin or ritonavir.

Dosage

ORAL
Acute treatment of nausea & vomiting:
Adult: 10-20mg every 4-8 hrs. Max 80mg/day.
Child: >2 yr & > 35 kg: 10-20 mg 3-4 times daily. Max 80mg daily.
Migraine:
Adult: 20 mg every 4 hours, in combination with paracetamol, as required. Max: 4 doses in 24 hours.
RECTAL
Acute treatment of nausea & vomiting: Adult 60 mg bid.
Child: 60 mg bid.
