Cefprozil, sometimes spelled cefproxil and sold under the brand name Cefzil, is a cephalosporin type antibiotic. In Europe, it is sold by the name Procef. It can be used to treat bronchitis, ear infections, skin infections, and other bacterial infections. It comes as a tablet and as a liquid suspension.

Absorption
Cefprozil is about 95% absorbed. Food increased T max 0.25 to 0.75 h. T max is about 1.5 h. C max is 18.3 mcg/mL.
Distribution
Vd is about 0.23 L/kg. Cefprozil is about 36% protein bound.
Elimination
Plasma t ½ is 1.3 h. Total body Cl is about 3 mL/min/kg. Renal Cl is about 2.3 mL/min/kg. The 8-h urinary excretion accounted for 54% to 62% of the dose.
Special Populations
Renal Function Impairment
The t ½ may be up to 5.2 h.
Hepatic Function Impairment
The t ½ increases to about 2 h.
Elderly
AUC is about 35% to 60% higher.

Cefprozil Adverse Reactions / Cefprozil Side Effects

Nausea; vomiting; diarrhoea; hypersensitivity reactions; nephrotoxicity; convulsions; CNS toxicity; hepatic dysfunction; haematologic disorders; pain at Inj site (IM); thrombophloebitis (IV infusion); superinfection with prolonged use.
Potentially Fatal: Pseudomembranous colitis.

Precautions

Prescribing Cefprozil in the absence of proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to provide benefit to the patient and increases the risk of the development of drug-resistant bacteria.
In patients with known or suspected renal impairment, careful clinical observation and appropriate laboratory studies should be done prior to and during therapy. The total daily dose of Cefprozil should be reduced in these patients because high and/or prolonged plasma antibiotic concentrations can occur in such individuals from usual doses. Cephalosporins, including Cefprozil, should be given with caution to patients receiving concurrent treatment with potent diuretics since these agents are suspected of adversely affecting renal function.
Prolonged use of Cefprozil may result in the overgrowth of nonsusceptible organisms. Careful observation of the patient is essential.
If superinfection occurs during therapy, appropriate measures should be taken.
Cefprozil should be prescribed with caution in individuals with a history of gastrointestinal disease particularly colitis.
Positive direct Coombs’ tests have been reported during treatment with cephalosporin antibiotics.

Special Precautions

Hypersensitivity to penicillins; renal impairment; porphyria. Monitor renal and haematologic status. Pregnancy, lactation.

Other Drug Interactions

Renal excretion delayed by probenecid. Decreases contraceptive effect of oestrogen-containing oral contraceptives.

Dosage

Oral
Skin and soft tissue infections, Susceptible infections
Adult: 500 mg/day as a single dose or in 2 divided doses, increased to 500 mg bid if necessary.
Child: 20 mg/kg/day once or twice daily, up to a max dose of 500 mg once daily, if necessary, bid for otitis media.

Food(before/after)

May be taken with or without food. (May be taken w/ meals to reduce GI discomfort.)

Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters).
