Chlorzoxazone is a centrally acting muscle relaxant used to treat muscle spasm and the resulting pain or discomfort. It acts on the spinal cord by depressing reflexes. It is sold as Muscol or Parafon Forte, a combination of chlorzoxazone and acetaminophen. Possible side effects include dizziness, lightheadedness, malaise, nausea, vomiting, and liver dysfunction. Used with acetaminophen it has added risk of hepatoxicity, which is why the combination is not recommended.

Absorption
T max is 1 to 2 hours.

Metabolism
Rapidly metabolized.

Elimination
Excreted in the urine as glucuronide conjugate. Less than 1% is excreted in the urine as unchanged drug. The t ½ is about 60min.

CNS
Drowsiness; dizziness; lightheadedness; malaise; overstimulation.

Dermatologic
Allergic-type skin rashes; petechiae.

GI
GI disturbances.

Urine discoloration (orange or purple red).

Hepatic
Drug-induced hepatitis.

Miscellaneous
Hypersensitivity (eg, angioneurotic edema; anaphylaxis).

Hepatic Function
Avoid use in patients with liver impairment; discontinue if signs of dysfunction occur.

Hazardous Tasks
May impair ability to perform tasks requiring mental or physical coordination or dexterity, including driving. Drowsiness is very common.


Overdosage

Symptoms
Nausea, vomiting, diarrhea, drowsiness, dizziness, lightheadedness, malaise, sluggishness, loss of muscle tone, decreased deep tendon reflex, respiratory depression.

Dosage

Adults
PO Initial (for acute pain): 500 mg 3 or 4 times daily; increase to 750mg if needed. As improvement occurs, reduce dose; 250 mg 3 or 4 times daily is usually sufficient.
