Lanthanum carbonate, La2(CO3)3, is the salt formed by lanthanum(III) cations and carbonate anions. It is an ore of lanthanum metal, along with monazite.

Pharmacokinetics

Lanthanum carbonate is a non-calcium, non-aluminium phosphate binder used for hyperphosphataemia in patients with chronic renal failure. It works by inhibiting phosphate absorption by forming highly insoluble lanthanum phosphate complexes, thus reducing both serum phosphate and calcium phosphate product.

Highly bound to plasma proteins.

Metabolism
Not metabolised.

Excretion
Eliminated mainly by the biliary system. Elimination half life: Approx 36 hours.

GI disturbances, including nausea, vomiting, constipation, diarrhoea, dyspepsia and abdominal pain.

Precautions

Use with caution in patients with acute peptic ulcer, ulcerative colitis, Crohn disease, or bowel obstruction because these populations were not included in clinical studies.

Supportive treatment is recommended.

Dosage

Oral
Hyperphosphataemia in end stage renal failure
Adult: Initially, 0.75-2.25 g daily, given in divided doses wth meals. Dose may be adjusted every 2-3 weeks until an acceptable serum phosphate concentration is achieved; usual maintenance dose: 1.5-3 g daily in divided doses. Max: 3.75 g daily.

Food(before/after)

Should be taken with food. (Take with or immediately after meals. Chew thoroughly before swallowing. Do not swallow whole.)
