Flunisolide (marketed as AeroBid, Nasalide, Nasarel) is a corticosteroid often prescribed as treatment for allergic rhinitis.

The principal mechanism of action of flunisolide is to activate glucocorticoid receptors. It also has anti-inflammatory action.

Flunisolide exerts glucocorticoid activity exhibiting potent anti-inflammatory and immunosuppressive effects, at least partly through inhibition of the release of various cytokines.

Absorption

Absorbed following intranasal administration.

Metabolism
Extensive 1st pass metabolism (only 20% of dose systemically available); converted to 6β-hydroxyflunisolide (has little glucocorticoid activity).

Excretion
4 hours (elimination half-life).

Osteoporosis; muscle wasting; hyperglycaemia; impaired tissue repair; delayed wound healing; amenorrhoea, hyperhidrosis; ocular changes; mental and neurological disturbances; increased susceptiblity to infections.

Precautions

Monitor
Document disease state activity and/or symptoms before starting therapy and periodically during treatment. Notify health care provider if symptoms do not improve or worsen. Ensure that therapy is periodically reviewed to determine if therapy needs to be continued without change or if a dose change (eg, increase, decrease, discontinuation) is indicated.

Special Precautions

Congestive heart failure; recent myocardial infarction; hypertension; diabetes mellitus; epilepsy; glaucoma; hypothyroidism; osteoporosis; peptic ulceration; psychoses; renal or hepatic impairment; elderly; childn; pregnancy and lactation.

Other Drug Interactions

GI bleeding and ulceration with NSAIDs. Decreased serum concentrations of salicylates. Decreased effect with barbiturates, carbamezepine, pheytoin, primidone or rifampicin.

Potentially Fatal: Hypokalaemia with potassium-depleting drugs such as thiazide, furosemide or amphotericin B.

Treatment and prophylaxis of allergic rhinitis
Adult: Per metered spray contains 25 mcg: Initially, 2 sprays into each nostril bid, may increase to tid if needed, then reduce for maintenance.
Child: 5-14 years: Per metered spray contains 25 mcg: 1 spray into each nostril up to tid.

Inhalation
Asthma
Adult: As a metered-dose aerosol using chlorofluorocarbon (CFC) propellant: 500 mcg bid up to 2 mg/day in severe asthma.
Child: >6-15 years: As a metered-dose aerosol using chlorofluorocarbon (CFC) propellant: Not to exceed 500 mcg bid.

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus.

Flunisolide and Children

Safety and efficacy of AeroBid in children younger than 6 yr of age not established. Safety and efficacy of Aerospan in children younger than 4 yr of age not established. Oral corticosteroids may suppress growth in children and adolescents, particularly with higher doses over extended periods. Routinely monitor growth of children receiving inhaled corticosteroids.

Flunisolide and Geriatic

Dose selection should be cautious, usually starting at the lower end of the dosing range, reflecting the greater frequency of decreased hepatic and renal function, and comorbidity.

Flunisolide and Other Contraindications

Hypersensitivity; presence of untreated acute infections; active or quiscent tuberculosis; ulcerative conditions.
