Losartan+Hydrochlorothiazide belong to angiotensin II antagonist and low-ceiling thiazide diuretics, respectively. Losartan and Hydrochlorothiazide are a fixed-dose combination is indicated for the management of hypertension in adults who are unresponsive to other therapeutic options. Losartan and Hydrochlorothiazide are also prescribed to reduce the risk of stroke in hypertensive patients with LVH.

Pharmacodynamics

As a diuretic, Hydrochlorothiazide increases urinary excretion of sodium and chloride salts and reduce cardiac load. Losartan, an angiotensin II receptor antagonist (ACE-II inhibitor) blocks the action of ACE-II and lowers blood pressure. Losartan selectively blocks the effects of angiotensin II including vasoconstriction and aldosterone secretion. The additive effects of Losartan and Hydrochlorothiazide exerts additive effects against hypertension.

Losartan and Hydrochlorothiazide cause dry mouth, pancreatitis, anorexia, nausea, glycosuria, impotence, jaundice, constipation, thirst, electrolyte imbalance, palpitations, leucocytopenia, taste changes, dyspepsia, angioedema, cough and aggravation of SLE, hypotension, bronchitis, renal failure, interstitial nephritis, migraine, hyperlipidemia, sinusitis, aplastic anemia and bronchitis. Rarely, the drug combination can cause serious anaphylaxis, toxic epidermal necrolysis and haemolytic anemia.

Precautions

Losartan and Hydrochlorothiazide are contraindicated in patients with electrolyte imbalance, hyperuricemia, renal artery stenosis, hepatic disorders, hyperlipidemia, diabetes, gout, hypovolemia, parathyroid dysfunctions and mitral stenosis.

Herbs such as Yohimbe, garlic, St. John’s wort, Dong Quai, ephedra and ginseng may impair the metabolism of Losartan and Hydrochlorothiazide.

Dosage

For hypertension (Adults):
Consider oral administration of one tablet (Losartan 50 mg and Hydrochlorothiazide 12.5 mg) daily. After 2-4 weeks of initial therapy, the dosage can be increased twice daily. The maximum dose should not exceed 2 tablets/day.
