Valganciclovir, an antiviral drug, is prescribed for the treatment of cytomegalovirus infection (CMV) of eye in adults with AIDS. Valganciclovir can be used to prevent CMV infection in adults and children who undergone organ transplantations such as pancreas, kidneys and heart. The drug lowers the symptoms by preventing the viral replication. However, Valganciclovir should not be used to cure CMV in adults and neonates of an infected mother. Valganciclovir is an alternate to Ganciclovir.

Pharmacodynamics

Valganciclovir is a prodrug, and an L-valyl ester of Ganciclovir. After oral administration, Ganciclovir is phosphorylated by viral protein kinase and then by cellular kinases. Ganciclovir is an analogue of 2'-deoxyguanosine that can prevent CMV replication.

Pharmacokinetics

After oral administration, the drug is well absorbed in GIT and rapidly metabolized in the intestinal wall and liver to a drug metabolite, Ganciclovir. The oral bioavailability of the metabolite is 60%. The AUC and Cmax are influenced by high-fat meals intake. The primary route of elimination of Valganciclovir is by kidneys with a renal clearance rate of 2.99± 0.67 mL/min/kg. The terminal half-life of Ganciclovir is 4.08± 0.76 hours.

Valganciclovir Adverse Reactions / Valganciclovir Side Effects

Valganciclovir can cause seizure, light-headedness, rapid heartbeats, troubled breathing, fever, chills, seizure, weight gain, diarrhoea, tremors, drowsiness, easy bruising, urination changes, flu-like symptoms, anorexia, polydipsia, GI changes and pale skin.

Precautions

Valganciclovir should not be prescribed to dialysis-dependent renal disorder patients, liver diseases, anemia and patients on corticosteroids or radiation therapy. Valganciclovir can impair thinking and mental alertness. Do not drive vehicles or operate heavy machineries while taking Valganciclovir. Avoid close contact with sick/infected people. Avoid outdoor activities that can cause injury or bleeding risks.

Dosage

For the treatment of CMV retinitis (Adults):

The recommended induction dose is 900 mg in two divided doses for 21 days. For maintenance dose after induction in patients with inactive CMV retinitis, the recommended dosage of Valganciclovir is 900 mg, once daily.
Prevention of CMV Disease

For pancreas or heart-transplanted patients, the recommended dose is 900 mg once daily. The treatment should be started within ten days after transplantation and should be continued up to 100 days, post-transplantation. For renal transplanted patients, the post-transplantation duration should be continued up to 200 days.
 
For the prevention of CMV Disease (Pediatrics):

For renal or heart-transplanted children (4 months to 16 years of age), Valganciclovir should be taken as once-daily. The treatment should be started within ten days after transplantation and should be continued up to 100 days, post-transplantation. The dosage is based on creatinine clearance and body surface area calculations, as prescribed.

Valganciclovir and Pregnancy

USFDA pregnancy category C. May be or may not be harmful to an unborn fetus. Consult a physician if you are in gestation or planning to have a baby during Valganciclovir treatment. Use effective contraceptive mode at least for 30 days after the discontinuation of Valganciclovir. Valganciclovir can affect fertility in both sexes. Valganciclovir should not be used to prevent vertical transmission in HIV mother.

Valganciclovir and Lactation

Avoid breastfeeding while taking Valganciclovir. Breastfeeding is not advisable in HIV/AIDS mothers.
