Ibandronate, a bisphosphate group of drug, is prescribed for the therapeutic management and prevention of osteoporosis in postmenopausal women. Ibandronate interferes with bone metabolism including bone formation and breakdown. Ibandronate delays bone loss, increase bone mass and prevent pathological bone fractures.

Pharmacodynamics

Ibandronate induce dose-dependent biochemical changes that inhibit bone resorption, bone collagen degradation in recommended doses. Ibandronate can lower bone turnover, serum osteocalcin levels, urinary C-terminal telopeptide of Type-I Collagen (uCTX) in postmenopausal women.

Pharmacokinetics

Oral administration of Ibandronate occurs in GIT and the plasma concentrations increase in a dose-linear manner. The peak plasma concentration can be achieved within 0.5 to 2 hours in the fasting state. The oral bioavailability is altered by food intake and beverages. After GI absorption, Ibandronate either binds with bone or excreted via urine. The terminal half life of Ibandronate is 90 L, and up to 99% of the drug is bound to plasma proteins. The metabolism is not via hepatic cytochrome P450 system, and elimination is via the urine as unchanged drug. The terminal half life of Ibandronate is 37-150 hours.

Ibandronate Adverse Reactions / Ibandronate Side Effects

Ibandronate can cause worsening of heartburn, chest pain, jaw pain, severe bone and joints disorders, unusual pain in the thigh or hip, myalgia, back pain, headache, diarrhoea, nausea, stomach upset, injection-site reactions and pain in arms and legs.
Some of the other serious and rare adverse events of Ibandronate are osteonecrosis of jaw, numbness, gum infections, jaw pain, loose teeth and gum pain.

Precautions

Ibandronate tablets should not be prescribed for patients with severe renal disease, esophageal disorders, vitamin-D deficiency, dysphagia, hypocalcemia and Ibandronate allergy.

Ibandronate can interact with all NSAIDs including piroxicam, celecoxib, ketoprofen, ibuprofen, indomethacin, naproxen, ketorolac and diclofenac.

Other Interactions

Mineral water, food, beverages, milk or milk products, antacids, vitamins and mineral supplements including calcium may interact with Ibandronate tablets. Avoid taking any of these for at least 60 minutes after taking Ibandronate tablet.

Dosage

Oral Ibandronate tablets can be taken either as one 150 mg tablet, once monthly on the same date of each month or 2.5 mg of Ibandronate tablet as once-daily dosage.
 
For Osteoporosis (Adults):
For daily oral doses, 2.5 mg of Ibandronate should be prescribed. As once a month dosage, 150 mg of Ibandronate should be taken on the same day of each month. Alternatively, 3 mg of Ibandronate should be administered intravenously over 15-30 every three months. Ibandronate should be taken only with plain water. Avoid taking the drug with beverage and mineral water.
 
For Prevention of Osteoporosis (Adults):
For daily oral doses, 2.5 mg of Ibandronate should be prescribed. As once a month dosage, 150 mg of Ibandronate should be taken on the same day of each month. Alternatively, 3 mg of Ibandronate should be administered intravenously over 15-30 every three months. Ibandronate should be taken only with plain water. Avoid taking the drug with beverage and mineral water.
